CITY OF ROSEVILLE <br /> FLEXIBLE BENEFITS <br /> REGULAR FULL-TIME EMPLOYEES <br /> (NON-UNION EMPLOYEES ONLY) <br /> EMPLOYEES MAY CHOOSE ONE OF THE FOLLOWING OPTIONS: <br /> Option A: Employee chooses to stay with the current arrangement <br /> whereby the City contributes up to $290 toward the <br /> premiums for health insurance, basic life insurance <br /> ($8,000) and long term disability for persons choosing <br /> family coverage. For employees choosing single <br /> coverage, the City contributes the actual cost of health <br /> insurance, basic life insurance and long term disability. <br /> Option B: Employee chooses a monthly city contribution toward <br /> deferred compensation of $130/month. The City pays <br /> the cost of long terns disability and basic life insurance. <br /> Requires proof of health insurance coverage in another <br /> program and agreement to remain covered during a <br /> plan year. <br /> Option C: Employee chooses a monthly cash benefit of <br /> $115/month. The City pays the cost of lung term <br /> disability and basic life insurance. <br /> Requires proof of health insurance coverage in another <br /> program and agreement to remain covered during a <br /> plan year. <br />
