You are inv ted to submit your Proposal for the administration of a Post <br />Employment Health Care savings Plan based on the information contained in <br />this Request for Proposal. Unless a specific note is made to the contrary, we will <br />assume that your Proposal conforms to the city's Specifications. We also expect <br />that your Proposal complies with all applicable state and federal laws. <br />The city recognizes that more than one vendor may be involved with the <br />submission of a particular Proposal. As noted in the summary of Requested <br />Information and Proposal Questionnaire, all vendors must be addressed. The <br />City reserves the right to independently evaluate each vendor and accept/reject <br />all or any of the vendors. <br />You are invited to ask questions during the proposal process and to seek <br />additional information, if needed. We want this to be an interactive process and <br />will make every effort to provide sufficient data for your response. <br />General Conditions and Stipulations <br />Employee census is correct and accurate to the best of our knowledge. All <br />vendors submitting Proposals will be provided information regarding changes <br />or additlbns to this information. This information will be provided on a uniform <br />and nondiscriminatory basis. <br />The City reserves the right to adept or reject any or all Proposals and to <br />waive formalities and select the vendor(s) offering the product and /or services <br />that best meet the needs of the city and its employees. The city's objective <br />is to select a vendor(s) who will provide the best possible service at the best <br />possible cost while meeting the Request for Proposal specifications. The city <br />is not obligated to award the contract based on cost alone. <br />A servicing representative must be available to the city initially and on an on- <br />going basis to assist with any meetings the city conducts to introduce the <br />Plan, for purposes of enrollment activities, to explain Plan changes, etc. <br />The vendor(s) awarded the business shall submit properly executed contracts <br />to the city within sixty (60) days of the Plan effective date. To the extent the <br />Plan effective date precedes the date of the contracts, services shall be <br />performed in accordance with a letter of understanding not inconsistent with <br />the Proposal. <br />The city will designate a project manager to coordinate the Plan on behalf of <br />the city and its employees. The successful vendor(s) will be required to work <br />closely with the designated project manager. Similarly, the successful <br />vendor(s) will be expected to identify an individual who will serve as the key <br />contact person and to specify other staff that will perform various tasks. <br />Page 6 <br />
