The program can be made available where at least three (3) eligible participants <br />apply for coverage. <br />Other Features <br />Portable. The policyholder can retain coverage with no increase in premium if <br />he or she changes jobs. <br />Offered to all full -time employees up to age 67 who work at least 20 hours per <br />week on a regular basis. <br />Guaranteed Renewable. The policy is guaranteed renewable to the }premium <br />due data on or next following the policyholders 701h birthday. <br />Premium rates are the same for males and ferrules. <br />Worldwide Coverage. The policyholder is covered for disabilities occurring <br />outside the regularly covered geographical areas. <br />Wellness rider. This optional rider will pay 60 per year for a policyholder <br />who has one of several specific health screening tests. <br />The ENHANCE policy is designed to provide coverage for disabilities <br />resulting from covered accidents or cornered sicknesses, subject to any policy <br />limitations. <br />_M111,11,44-DON <br />Total Disabrllty: During the first year, totally disabled or disabled means you <br />are unable to work at your jab and are not, in fact, working at your place of <br />employment for pay or benefits and are under the care of a doctor. <br />After the first year of disability, if applicable, totally disabled means you are <br />unable to work at any job for which you are qual ed by reason of education, <br />training or experience; not, in fact, working at any job for pay or benefits and <br />under the care of a doctor. <br />} <br />s <br />If you are not working when you become totally disabled, we will pay benefits <br />only as long as you are unable to perform all of the substantial and material <br />duties of you regular occupation, and are -under the care of a doctor. Under the <br />care of a doctor means your are being cared for on a regular basis by a doctor. <br />Your regular occupation means your last full-time job. <br />
