Filing a Vulnerable Adult Reoort <br />A common entry point must be availabfe 24 hours per day to take calls from reporters of <br />suspected maltreatment. <br />A reporter can choose to make the report ananymousEy. <br />When a reporter cafls they should be prepared with the fo!lawing information: <br />• The time, date and loca�ion of the incident. <br />• ThE names of the persons involved, including but not fimited to perpetrators, a[ieged <br />victim and wii:nesses. <br />o The addresses, phone numbers and birthdates of the persons involved. <br />e lNhether there is a risk Qf imminent danger �o the alieged victim <br />• The description of the suspected maltreatment. <br />• The disabii�ty, if ar�y, ofi the afleged victim <br />• The relationship of the alleged perpetrator to the alleged victim <br />• Whether a facility was involved and, if so, which agency licenses the facility. <br />• ;Nhether law �riforcement ��as been n�tifed. <br />• Whetf�er the repor�er vv�ishes'to receive r�oYification of the initial and final reports. <br />Initiat Disposition: "Initial disposition" is the �ead agency's determination o� whether the <br />report wifi be assigned for further investigation. A reporter can request to be notified of <br />the initial disposition of a case. r� the repor#er would like to be notified the lead agency <br />will provide the notifica�ion with a let�er. <br />Final Disposition; "Final disposition" is the determination of an investigation by a lead <br />agency that a report of maltreatment under Laws 1995, chapter 229, is substantiated, <br />inconclusive, false, or that no determination will be made. If the lead agency is the State <br />Department of Health or Human Services a reporter may also request to be notified of the <br />final disposition of a c�se. <br />Iden�ity of Reporter <br />The identity of the person making the maltreatmer�t report is confidentiai and can oniy be <br />released with the cor�sent of the reporter or wi�h a court order. <br />
