Page 2 V. DESCRIPTION OF ROSEWOOD ESTATE Rosewood Estate exemplifies the concept of a Residential Alternative to the traditional institutional nursing home environment for our frail elderly. By providing choices and options for personalized services, customers have the opportunity to maintain independence and dignity in their lives. By providing a secure residential setting for the frail elderly, Rosewood Estate develops the atmosphere for customers and families to use as many or as few of the services available to maintain independence in their lives and in their own "homes." The residents' family or an outside agency can provide service or assistance on a regular or temporary basis. A licensed Home Health Care agency (presbyterian Homes of Minnesota) offices in Rosewood Estate and contracts separately with each resident for services provided to the resident. These services are provided to residents as needed and are paid for in 15 minute increments or on a flat charges-for-service basis. Residents who live at Rosewood Estate sign a residential lease which renews with each successive month. As part of the base monthly rent, Rosewood Estate provides two meals per day, social activities, housekeeping, maintenance and professional management. Laundry can be done by the resident, family member or by the home health agency for a small fee. About 500.10 of the residents purchase a third meal. An array of cultural activities are available to residents as well as transportation to outside events and appointments. Eitdtty perrent of all residents use the services described above on a regular basis from the in.:Itouse home health care agency. Assistance in activities of daily living (i.e., dressing, JVooming, ambulating, eating, using the bathroom, and bathing) are provided by the mdependent licensed Home Health Care agency. Medication monitoring and other professional health care services are also available from the same agency. Although many residents receive scheduled assistance, all residents of Rosewood Estate have the availability of assistance with their needs by utilizing the "alert system" communication devices provided to every Rosewood Estate resident. Most residents at Rosewood Estate are women aged 80 or older. About 10% of the total resident population are couples. Nearly 50% of residents previously resided in a nursing home, 270/0 resided at home and 23% come from a hospital or other setting. Typically, 69% of the residents have previously lived within 7 miles of Rosewood Estate. The income profile of residents reflects that approximately 600.10 of residents have incomes of$20,OOO or less per year. The average resident will live at Rosewood Estate for 15.9 months. Discharge to a nursing home or death account for 77% of the turnover of Rosewood Estate residents. The balance leave to reside with family or others. Rosewood Estate is a Residential Living Alternative, as opposed to the traditional institutional alternatives, for people who need support with their daily activities and cannot live alone. In some cases, this support includes health care.
