<br />will be given opportunity to select vacation periods insofar as it <br />is practical. Selection of vacation time shall be based on service <br />credit. Vacation periods of five (5) days or less should be <br />arranged at least one week in advance. <br /> <br />252.090. Carry-over and Waiver of Vacation Leave. <br />No employee shall be permitted to carryover vacation time from one <br />year to the next. Provided that, if in the opinion of the City <br />Manager, unusual circumstances exist, he or she may approve a <br />carry-over of all or part of the previous year's vacation. <br /> <br />252.100. Sick Leave Plan. <br />Regular full-time employees may use sick leave for: <br />1. Personal illness or injury. <br /> <br />2. Personal medical or dental appointments. <br /> <br />3. Illness or injury of a spouse, child, parent or other <br />member of an employee's household which requires the <br />employee's care and attention. <br /> <br />4. Medical or dental appointments of a spouse, child, parent <br />or other member of an employee's household which require <br />the employee's care and attention. <br /> <br />252.110 Proof Reauired. <br />In order to be eligible for sick leave pay, an employee must: <br /> <br />1. Report promptly, to the supervisor, the reason for the <br />absence. <br /> <br />2. Keep the supervisor informed of the employee's condition if <br />the absence is more than three working days duration. <br /> <br />3. Submit a medical certificate for any absence exceeding three <br />working days, if required by the City Manager or designee. <br /> <br />4. Submit a medical certificate for absences of less than three <br />working days when the frequency or pattern of sick leave usage <br />indicates possible abuse of sick leave benefits. <br /> <br />252.120. Rate of Accumulation for Sick Leave. <br />Sick leave pay will be granted, according to accumulated earned <br />service credit from the date of employment, at the rate of one day <br />paid sick leave per month. Sick leave may be accumulated to an <br />unlimited amount. <br /> <br />252.130. Application of Workers' Compensation Benefits to sick <br />Leave Proaram. <br />In cases of absence where an employee receives compensation <br />benefits under Workers' Compensation, the total compensation <br />received from both Workers' Compensation and sick leave shall not <br /> <br />6 <br />
