M. <br />Proposal Submission <br />A. Valid Proposal <br />In order to be considered valid, an electronic version of the proposal will be submitted in PDF format. <br />The submittal shall at least include a letter of transmittal, a completed Respondent Offer — Signature and <br />Certification Form (Appendix A) and the proposal. These items shall include the following information: <br />1. Letter of transmittal: signed by the officer of the company who can be accountable for all <br />representations in the proposal. <br />2. Respondent Offer — Signature and Certification Form (Appendix A) <br />3. Electronic copy of the proposal: The proposal shall be limited to 25 pages (the other <br />required sections do not count against the 25 page limit). Proposals should be complete <br />and concise. The proposal must contain the following information, presented in the order <br />shown: <br />a) Consultant Team Description: Introduce the consultant team, define the role(s) of the <br />firm or firms involved with each of the various aspects of the construction of the <br />proj ect. <br />b) Key Staff: List key staff along with a brief statement of their respective role. <br />c) Proposed Work Plan and Tasks: Discuss specific project tasks with a brief discussion <br />of the recommended approach(es) to be taken. The scope of consultant work <br />necessary to achieve desired results should be identified. Specific techniques and <br />methodologies should be included in this section. <br />d) Project tasks. Respond to listed tasks in the RFP and identify any additional tasks <br />required for the successful delivery of this project and the construction of the <br />proposed improvements. For each task listed, identify: <br />• Specific staff to be involved, roles, responsibilities <br />• Time commitment for each person <br />• Estimated timeframe for each major task/element and project total <br />• Deliverables <br />• City responsibilities <br />e) Experience and Qualifications: Identify the general background, structure and <br />organization, and available resources personnel and experience of the participating <br />firm(s). Additional information should be included to demonstrate competence and <br />performance ability in similar projects. The project manager and other members of <br />the project team should be identified with relevant information concerning training <br />and experience, which prepares them for the particular project responsibilities. <br />I) Fees (Attachment B) <br />g) Resumes of key project participants, including prior projects of similar size and scope <br />for which the participants played the same or a similar role as proposed for this <br />proj ect. <br />4. Notice to Proposer <br />a) The City is not responsible for costs incurred by anyone responding to this Request <br />for Proposals. <br />b) Upon submission, all proposals become the property of the City, which retains the <br />right to use any concept or idea presented in any proposal submitted, whether or not <br />that proposal is accepted. <br />Page 11 <br />
