Transmit final documents in pdf form to Roseville staff. <br /> <br />TASK 3 – REGULATORY APPROVAL (June 2024) <br />For this task, SEH proposes to engage the Minnesota Department of Health (MDH)to seek approval of the <br />proposed Meter Vault Replacement Project. Our many years of working with the MDH leads us to plan on the <br />following activities: <br />1. Pre-Design Meeting <br />a. Hold a virtual meeting with the appropriate MDH staff to introduce the project, and <br />b. Solicit input to support project approval. <br />2. Final Design <br />a. Upon Roseville approval submit two (2) hardcopies and one (1) pdf copy of the completed <br />documents to the MDH for review and approval. <br />b. Hold a virtual meeting with the appropriate MDH staff to address any questions or concerns they <br />may have, and <br />c. Submit follow-up materials as may be needed to allow the MDH to grant approval of the proposed <br />project. <br /> <br />PROJECT DELIVERABLES <br />The project deliverables, also defined in the Task descriptions above, include electronic copies of the documents <br />generated for each Task. <br /> <br />PROJECT SCHEDULE <br />The project schedule as defined in the Tasks above assumes a notice to proceed is received on, or about May <br />17, 2024: <br />Task 1 – Project Initiation & Data Collection May 2024 <br />Task 2 – Meter Vault Design June 2024 <br />Task 3 - Regulatory Approval June 2024 <br /> <br />PROJECT ASSUMPTIONS <br />The following assumptions have been incorporated into the development of this professional services <br />proposal: <br />1. Design services requested by Roseville are limited to the production of construction plans and technical <br />specification sections in CSI Divisions 2 through 44. The City of Roseville will incorporate the <br />construction plans and technical specification sections into the City’s bidding and contracting <br />documents.(An option to assemble a complete set of bidding documents is noted in the Compensation <br />section of this proposal.) <br />2. No survey activities are required by SEH. Roseville will provide all necessary site survey data to SEH. <br />3. No temporary or permanent easement acquisition work or any associated easement survey, or plat <br />work is required by SEH. <br />4. No environmental contamination assessment for soil or building material sampling and testing work <br />has been included in this proposal. <br />5.
