<br />(1) Form. Every application for a license under this ordinance shall state <br />the name of applicant, his age, representations as to his character, with such <br />references as the Council may require, his citizenship, the type of license applied <br />for, the business in connection with which the proposed license will operate and <br />its location, whether the applicant is owner and operator of the business, how <br />long he has been in that business at that place, and such other information as the <br />Council may require from time to time. In addition to containing such <br />information, the application shall be in the form prescribed by the liquor control <br />director and shall be verified and filed with the City Clerk. No person shall <br />make a false statement in an application. From time to time, at the request of <br />the City Manager, a licensee will provide data to the City concerning that <br />portion of its revenue attributable to the sale of food and the sale of liquor <br />and/or wine. <br /> <br />(2) Liability Insurance. Prior to the issuance of a license under this <br />ordinance, the applicant shall file with the City Manager an insurance policy <br />covering liquor liability and loss of means of support in the amount of one <br />hundred thousand dollars ($100,000.00) coverage for one person and two hundred <br />thousand dollars ($200,000.00) coverage for more than one person and providing <br />for ten thousand dollar ($10,000.00) coverage for property damage and complying <br />with the provisions of Minnesota Statutes Section 340.00, relating to liability <br />insurance policies. <br /> <br />(3) Approval of Insurance. Liability insurance policies shall be approved <br />as to form by the City Attorney. Operation of a licensed business without having <br />on file with the City at all times an insurance policy as required in Subdivision 2 <br />is a cause for revocation of the license. All insurance policies shall state that <br />the City will be given ten days notice, in writing, of cancellation. <br /> <br />(4) Bond. Prior to the issuance of a license under this Chapter, the <br />applicant shall file with the City Clerk a bond with corporate surety or cash or <br />United States Government bond in a sum of three thousand dollars ($3,000.00) for <br />off-sale licenses and five thousand dollars ($5,000.00) for other licenses to be <br />used as provided for in and complying with Minnesota Statutes Section 340.12, <br />relating to such bonds. <br /> <br />151.040. License Fees. <br /> <br />(1) The annual license fee for an "on-sale" intoxicating liquor license is <br />seven thousand dollars ($7,000.00). The annual license fee for a wine license is <br />seven hundred and fifty dollars ($750.00) for establishments with seventy-five <br />(75) seats or less and one thousand five hundred dollars ($1,500.00) for <br />establishments with over seventy-five (75) seats. The annual license fee for a <br />Sunday "on-sale" license is two hundred dollars ($200.00).
