The plan will use or disclose PHI and EPHI for any <br />other purpose only upon receipt of an authorization from the <br />individual. The Plan will not disclose "protected health <br />information" (PHI) or "electronic protected health information" <br />(EPHI) to the Employer until the Plan has received certification <br />from the Employer that the plan documents have been amended to <br />incorporate the following provisions: As Employer the Employer <br />agreesto: <br />(i) not use or further disclose "protected health <br />information" (PHI) or "electronic protected health <br />information" (EPHI) other than as permitted or required by <br />the plan document or as required by law; <br />(ii) ensure that any agents, including a <br />subcontractor, to whom it provides PHI or EPHI agree to <br />the same restrictions and conditions that apply to the <br />Employer with respect to PHI or EPHI and will implement <br />reasonable and appropriate security measures to protect the <br />information; <br />(iii) not use or disclose the PHI or EPHI for <br />employment-related actions and decisions or in connection <br />with any other benefit or employee benefit plan of the <br />Employer; <br />33 City of Roseville MPD <br />
