(iv) be vigilant of any use or disclosure of PHI or <br />EPHI and report to the plan any use or disclosure that is <br />inconsistent with the permitted or required uses or <br />disclosures and any security incident of which it becomes <br />aware; <br />(v) make available PHI or EPHI to individuals; <br />(vi) provide individuals with the opportunity to <br />amend PHI or EPHI; <br />(vii) provide individuals with an accounting of <br />the disclosure of their PHI or EPHI; <br />(viu) make its internal practices, books, and <br />records relating to the use and disclosure of PHI or EPHI <br />available to the Secretary of the Department of Health and <br />Human Services; for compliance purposes; <br />(ix) return or destroy all PHI or EPHI, if feasible, <br />if not feasible then limit further uses and disclosures to <br />those purposes that make the return or destruction <br />infeasible; <br />(x) ensure that adequate separation exists <br />between employees who are authorized to use PHI or EPHI <br />and those who are not; <br />34 City of Roseville MPD <br />
