(xi) ensure that adequate separation is supported <br />by reasonable and appropriate security measures to protect <br />the information; <br />(xii) describe those employees or classes of <br />employees to be given access to PHI or EPHI; <br />(xiii) restrict the access to and use by these <br />employees; <br />(xiv) provide an effective mechanism for <br />resolving any issues of noncompliance by persons who <br />have access to PHI or EPHI. <br />(j) In accordance with the HIPAA Privacy rules and <br />Security Standards, the following employees, or class of <br />employees, will have access to PHI or EPHL• employees with <br />Medical Reimbursement Plan oversight responsibility. <br />(k) The individuals described in (j) above will have <br />access to use and disclosure of PHI or EPHI for plan administration <br />functions that are performed by the Employer for the Plan. <br />(1) The Employer will provide a mechanism for <br />resolving noncompliance issues for any individual described in (j) <br />above who does not comply with the plan document, or improperly <br />uses or discloses PHI or EPHI, including disciplinary procedures. <br />35 City of Roseville MPD <br />
