This permit applies only to those projects that are designed under the supervision of a registered professional <br />engineer. Any project not meeting all conditions of the permit or any project the Department identifies as having the potential for <br />significant resource impacts, is not authorized herein. Rather, such projects must be submitted as separate individual permit <br />applications. <br />2. Erosion Control. All exposed areas above the ordinary high water level shall be sodded, seeded, or riprapped as needed to prevent <br />erosion and sedimentation. Erosion control measures shall be adequately designed for site characteristics. They may include staked <br />hay bales, diversion channels, sedimentation ponds, or sediment fences. They shall be installed prior to commencement and <br />maintained throughout the project All exposed soil shall be restored by seeding and mulching or sodding and staking within 72 hours <br />of completion of project The erosion control fence next to the water body shall run parallel to the contours and shall be trenched into <br />the soil to prevent underfloor. <br />3. Control sediment during construction. A sediment bamer shall be installed around the area of excavation/fill placement during the <br />project to limit turbidity to the affected area. This barrier may involve use of a filter fabric material attached to stakes or a snow fence <br />anchored to the bed of the public water. The barrier shall be removed upon completion of the work after collected sediment has been <br />removed to the greatest possible extent. <br />4. Flotation Sediment Curtain. Any work below the water level shall be encircled by a floatation sediment curtain to prevent sediment <br />from being transported beyond the construction site. This curtain shall be constructed and maintained as illustrated on the attachment <br />entitled "6.6 Flotation Silt Curtain". The bamer shall be removed upon completion of the work a$er collected sediment has been <br />removed to the greatest possible extent <br />S. Seedin¢. In the event the site cannot be stabilized with vegetation before October 15th, all exposed soil shall be adequately mulched <br />at a rate of not less than 35001bsJacre leaving no more than 20 percent visible soil surface and maintained until seeding/sodding can <br />be achieved <br />6. Preserve natural vegetation. Work done under this pemut must minim;~e impacts to natural shoreline and lake/stream bed vegetation. <br />7. Aquatic vegetation removal. No removal or destn-ction of aquatic vegetation is permitted unless an Aquatic Plant Management Permit <br />is obtained from DNR Fisheries Section(772-7950), or the removal consists of mechanical (i. e., not chemical) removal of submerged <br />or floating aquatic vegetation that meets the conditions for not requiring a pemut (i.e., not mechanical removal of submerged or <br />floating aquatic vegetation in an area of less than 2,500 square feet). Note: Removal of any amount of emergent vegetation is <br />prohibited unless a permit has been obtained from DNR Fisheries. <br />8. Purple Loosestrife. The permittee shall monitor all disturbed areas for the presence of purple loosestrife and control the plant as <br />descnbed in the suached pamphlet titled Purple Loosestrife Alert. The DNR aquatic plant manager should be contacted at 772-790 <br />for permit needs and additional information. <br />9. Structures. Structures shall not be constructed with materials that would have a detrimental impact on aquatic organisms or water <br />quality. <br />10. Material storage. Excavated materials must be deposited or stored in an upland area, in a manner where the materials will not be <br />redeposited into the protected water by reasonably expected high water or runoff. <br />11. 1F ,plau3, Excavated materials shall not be perrnanently placed within community designated flood plain areas or shoreland areas, <br />unless all necessary local permits and approvals have been obtained. <br />
